In order to respond to a patient report, a Health Care Institution may obtain a patient's name and contact information; however, in their response report, a Health Care Institution may not reveal the name or contact information of the patient to whom they are responding.
We strive to protect the privacy and identity of all health care professionals. The names of health care professionals may not be revealed by patients when reporting their experiences at a health care institution.
